Department: Community Based Services (CBS) Job Status: Full-Time
FLSA Status: Non-Exempt Reports to: Targeted Case Manager Team Leader
Positions Supervised: None Amount of Travel Required: 10-20%; Shawnee County Work Schedule: Monday thru Friday
Varied hours between 7 am to 7 pm; must be able and willing to adjust work schedules based on their clinical case load and the needs of individual clients.
ESSENTIAL FUNCTIONS
- Provide direct services to serious emotional disturbance (SED) children/youth and their families.
- Provide direct services based on the philosophies and principles of the strengths model.
- Complete required trainings for position.
- Complete required background checks every two years.
- Work collaboratively with the client, family, other collaterals and the treatment team to identify treatment goals, objectives and discharge criteria. This includes facilitating the treatment team planning process including the documentation of the individualized treatment plan, developing goals and objectives based upon the strengths assessment, monitoring to ensure the success of the treatment plan (which includes the follow-up with the child/youth and family on appointments and notifying treatment team members of treatment goal attainment), and making changes in the plan when needed. It is expected that this process includes the participation of the child/youth and family and other natural supports and collaterals.
- Ensuring collateral contact/updates occur through phone and written correspondence, face-to-face contacts with other social service agencies, schools, housing, employment resources, medical services and with all FSGC provider staff.
- Promoting individuals access to needed medical, social, educational and other needed services. In addition, the service assists with the application for benefits, and the arrangement of transportation to needed services.
- Coordination of services for children/youth as identified in an individual’s plan that has as its objective the assurance of an integrated, comprehensive plan of supports and services which includes family members, natural supports, and relevant community providers.
- Services provided to children and youth are to be coordinated with family.
- Assist youth and families with crisis planning when appropriate.
- Act as an advocate for children/youth and families. This will include assisting the client and family in accessing needed resources and additional services.
- Provide coordination of multiple services for youth and families.
- Participation in meetings for the purpose of case review, case assignments, and individual case planning. Targeted Case Managers’ caseloads shall be assigned and monitored by appropriate Team Leaders. The size of the caseload shall be assessed based upon clinical needs of the children/youth and family.
- Participate in internal and external networking activities designed to increase the likelihood of meeting the client and family’s identified needs.
- Targeted Case Managers are responsible for completing a case management treatment plan within 30 days of the acceptance of a family for case management services. Case management treatment plans are to be based on previously completed strengths and needs assessment.
- Complete clinical documentation (progress notes, etc.) in a timely fashion. Targeted Case Managers are also expected to stay informed of agency developments by reading memorandums and other agency related written communication.
- Respect and demonstrate ability to maintain client confidentiality.
- Act as an ambassador of Family Service and Guidance Center in the community setting. This may include doing public educational and informational presentations.
- Supports and actively promotes the vision and mission of Family Service and Guidance Center and the philosophy of the strengths perspective.
- Other duties as assigned by respective team leader or supervisors.
POSITION QUALIFICATIONS
Education/Licensure:
- BA/BS degree in human service field or 4 years of education/experience in a human service field
- Valid driver’s license, current auto insurance, a good driving record and a reliable vehicle are required

Other Physical Requirements:
Targeted Case Manager’s must have sufficient ambulatory capabilities to work with children and adolescents in the office setting. Position may require the ability to use physical holding (MAB) with children and adolescents exhibiting out of control behaviors.
WORK ENVIRONMENT
Office and Community-Based working environment; requires flexible schedule; frequent interruptions occur; expected to deal with individuals in crisis; risk of exposure to aggressive and/or acting out (verbal and physical) children and youth, and exposure to low socioeconomic homes and neighborhoods.
